Make Font Larger Health Plus Provider Directory> Select Your Directory

STEP 1 Select the type of directory you would like to create. You may either create an address based directory, a borough/county based directory, or a zip code based directory. Address based directory - This directory will include providers based on the address and mileage radius you specify. Borough/County based directory - This directory will include providers in the boroughs/counties you select. Zip Code based directory - This directory will include providers in the zip codes you select.
