General Help

Mobile Provider Finder allows you to search for health care providers. You can search for health care providers by name or by location. You can further refine your search by provider type, specialty, gender, other languages spoken, accepting new patients and ability to serve as PCP. Not all of these search options are required. Required fields are noted with *.

Once you find your health care provider, Mobile Provider Finder can call your health care provider, get directions to your health care provider, or add your health care provider to your contacts list. (Note: Adding providers to the contact list may not work on all mobile devices.)

Plan Selection Page

Select the plan in which you would like to search for providers. This selection is required when using the Mobile Provider Finder tool. Once you select the plan you wish to search from the dropdown, click the NEXT button to continue. Or click the BACK button to go back to the State Selection page.

Location Search or Name Search

Select the type of search you would like to execute. Location search allows you to search for providers of a specific provider type and specialty category within a specified geographic area. Name search allows you to search for a provider by name.

The BACK button will take you back to the Plan Selection page.

Location Search Page

When searching for a provider via the Location Search, you must select a provider type. Depending on the plan you are searching, you may see up to 7 provider types – Physicians; Other Health Professionals, Hospital, Facilities, Services and Equipment; Dental Provider; Vision Providers; Behavioral Health; and Pharmacies.

Next, you must enter the geographic area you wish to search. At a minimum, you must enter a city and state or a zip code. To get the most accurate results, enter a full address.

The mileage dropdown allows you select the radius of the search. For example, if you select 10 miles, Mobile Provider Finder will search 10 miles from the geographic location you entered.

Click the SEARCH button to execute your search. Or, click the MORE OPTIONS button to enter more search criteria. The BACK button will take you back to the page where you chose the Location or Name Search.

Name Search Page

When searching for a provider via the Name Search, you must enter a minimum of the first 2 characters of a provider’s last name or a facility name. This is a required field when performing a name search on the Mobile Provider Finder.

You have the option of entering the geographic area you wish to search. This information is not required, but if you wish to enter geographic information at a minimum, you must enter a city and state or a zip code. To get the most accurate results, enter a full address.

The mileage dropdown allows you select the radius of the search. For example, if you select 10 miles, Mobile Provider Finder will search 10 miles from the geographic location you entered.

You have the option of selecting a provider type when performing a Name Search. This field is not required. Additional Search Criteria Page

The items on this page are optional. You may use these fields to further refine your search results.

Able to serve as my PCP: Check this box if you only want to return Primary Care Physicians. This option will only display for certain plans and if “Physicians” or “No preference” was selected as the provider type. This is an optional field.

Accepting new patients: Check this box if you only want to return providers who are accepting new patients. This checkbox will only be displayed for some plan and provider type combinations. This is an optional field.

Specialty category: This dropdown allows you to further refine your search criteria. This is an optional field.

Gender:  This is also an optional field. This field allows you to narrow your search by looking for only male provider or only female providers. You can search for all providers by selecting the No Preference default value.

Other Languages:  You can search for providers who speak a language other than English. Select a language from the dropdown to narrow you search. This field is optional.

Click the SEARCH button to execute the search. The BACK button will take you back to the Location search page or the Name search page depending on which type of search you are executing.

Search Results Page

The search results page displays the name of the providers that match your search criteria. Up to 9 providers will be returned per page. A maximum of 100 providers will be returned. Clicking on the provider’s name will take you to the provider details page.

The MORE button will take to you the next page of provider search results. This button will only display if there are additional pages of search results. The PREVIOUS button will take to the previous page of the provider search results. This button will only display if you are on the 2nd page or further in the search results. The BACK button takes you to the previous search criteria page. The NEW SEARCH button allows you to start a new search from the state selection page.

Provider Details Page

This page shows you the details about the provider you selected. The provider’s name, address and phone number are shown. In addition, you’ll see the provider’s specialties and if applicable whether they are accepting new patients and/or able to serve as a Primary Care Physician.

Click the Call Provider link to call the provider. Click the Directions link to get driving directions to the provider’s office. Click the Add to Contacts link to add this provider to the contact list on your mobile device.

Note: See the Helpful Tips section for more information about how the Call Provider and Add to Contacts functionality may work differently on different mobile devices.

The BACK button takes you to the search results page. The NEW SEARCH button allows you to start a new search from the state selection page.

Starting Address Page

This page allows you to enter the starting location for the driving directories. A minimum of city and state or zip code is required. To get more accurate results, enter a full address.

Click the GET DIRECTIONS button to see the driving directions. Click the BACK button to go back to the provider details page.

Directions Page
This page shows you the directions from the starting address you entered to the provider’s location.

Click the Call Provider link to call the provider. Click the Add to Contacts link to add this provider to the contact list on your mobile device.

Note: See the Helpful Tips section for more information about how the Call Provider and Add to Contacts functionality may work differently on different mobile devices.

Click the BACK button to return to the Starting address page. This will allow you to change the starting point of the driving directions. Click the BACK TO SEARCH RESULTS button to return to the search results page. Click the NEW SEARCH button to start a new search on the state selection page.
